We’re looking for a passionate and dedicated Supports Coordinator to join our Clinical Team and help the people we serve reach their fullest potential.

What You’ll Do

As a Supports Coordinator, you’ll be the connection between individuals and the services that help them thrive. You will:

• Create and oversee person-centered plans to meet each individual’s unique goals
• Identify needs, dreams, and aspirations during the planning process
• Link and coordinate resources, supports, and services
• Collaborate on vocational profiles and employment plans
• Monitor, review, and adjust supports as needed
• Maintain accurate clinical case records and documentation
• Ensure compliance with consents, physicals, and other requirements

• Relationship Builder: You can connect with individuals and their support systems with empathy and professionalism
• Skilled Communicator: Strong verbal and written communication skills
• Culturally Sensitive: Understanding and respecting diverse backgrounds
• Tech-Ready: Comfortable with computer-based data entry
• Knowledgeable: Familiar with the Mental Health System and support services

Qualifications:

• Licensure as LSST, SST, LLBSW, LBSW, LLPC, LLP, LMSW, or MSW
• OR a Bachelors Degree in Human Services (social work, psychology, etc.)
•  STEP can help you obtain your LSST!

Schedule

• 7.5-hour shifts, Monday–Friday
• Must be able to commute to Wayne County, MI prior to start
